Senior Services
Recognized as one of the nation’s most innovative providers of all-inclusive medical and social services to the elderly with a specialty in dementia care and caregiver support, AltaMed will continue to make long-term senior care a top priority. This is particularly important considering that this population is expected to grow rapidly in the next decade in our services areas. Through our dedication to compassionate care, we are helping more seniors live with dignity in the comfort of their own home, and giving caregivers the respite and support they need to maintain a loving relationship with their aging parents or family members.

Our senior services include the innovative Senior BuenaCare program, nationally known as the Program of All-inclusive Care for the Elderly (PACE), home support for those who are home bound and require assistance with daily activities, care management as well as 3 adult day health care centers where seniors can receive medical care and enjoy the company of others, engage in games and participate in various arts and crafts.

Senior BuenaCare/Program of All-inclusive Care for the Elderly (PACE)

Senior BuenaCare/PACE is a managed-care program that uses an interdisciplinary team approach to provide patients with medical, social, nutritional and rehabilitative services. The Senior BuenaCare program makes it possible for those who are frail and meet nursing home eligibility criteria to live with dignity and pride in their own homes while receiving all the care they need.

• Hospitalization

Our program serves elderly people with multiple medical problems who:

• are 55 years old and over
• need special care and assistance
• would like to remain in their home
• are Medi-Cal and/or Medicare eligible or private pay
• live in the program service area

Home Care Services

The goal of our Home Care Services is to meet the needs of the elderly and home bound who require assistance with activities of daily life.

• Home Health Aides
• Certified Nursing Assistance
• Preparation of Hot Meals & Light Housekeeping
• Assistance with Personal Care and many more services

Care Management

AltaMed’s care management programs, including the Multipurpose Senior Services Program (MSSP) and the Integrated Care Management Program (ICMP) are designed to coordinate and provide services to frail and functionally impaired adults.

Adult Day Health Care (ADHC)

Our Adult Day Health Care centers are designed to help seniors enjoy an independent life while receiving the social and health care services they need in one comfortable enjoyable location. Our centers are also critical for caregivers.
