Patient Centered Medical Home

  • Improved coordination of care, access to services, and emphasis on self-management and education to improve health status and quality of life

  • Tracking and trending of HEDIS and IHA measures to improve on rates of preventive screenings and treatment of chronic diseases

  • Process and outcome metrics, analysis, and improvement

Wellness Promotion

Support and promotion of:

  • Health education
  • Outreach activities to assist and treat identified infirmed groups each month
  • Immunization campaign
  • Obesity program
